Please type your information directly on the form using Adobe Acrobat. Open the file and then click on the fields at the top of page one and type in your information, including your Certiport ID number if possible. Please remember to sign page 3 by hand.

Different Spellings
There are two different versions of the logo. The U.S. English version uses the spelling “Authorized Testing Center.” The international English version uses the spelling “Authorised Testing Centre.”

Please submit the version of the logo agreement that matches the logo spelling you wish to use.
